Diving Health Questionnaire
SCUBA diving requires a reasonable degree of health & fitness. We do ask all clients intending to participate in diving activities to review the standard PADI medical statement at the time of booking, as certain conditions may require you to seek medical advice as to whether you are fit to dive. Obviously any issues are better addressed with your own G.P. before you arrive, as if you are unable to complete the self-declaration form on the day you will be not be permitted to dive. If you answer Yes to any of the questions, you need to seek consent from your doctor before you can dive. If you are unsure then please seek appropriate medical advice before booking a diving activity to avoid disappointment if the dive centre declares it is unsafe for you to dive.
